What is mySRH?
mySRH is our secure, online electronic health record for managing and receiving your child's health information. With mySRH, you can use the Internet to see:
- Request medical appointments.
- View upcoming and past appointments.
- View your health summary from the mySRH electronic health record.
- View test results and clinical notes.
- View after visit summaries.
- Request prescription renewals.
- Access trusted health information resources.
- Communicate electronically and securely with your medical care team.
How do I sign up?
Patients who wish to participate will be issued a mySRH activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during your next office visit.

Return to TopHow do I locate clinical notes and results in mySRH?
- Clinical notes can be found under Visits and Appointments and Visits.
- Lab and Radiology Results are under Health Menu and Test Results.
- After Visit Summaries can be found under Appointments and Visits and Document Center.
When can I review visit information in mySRH?
- Once the visit is complete, some information is immediately available:
- Clinical Notes - as soon as the provider has completed their assessment and notes.
- Lab and Radiology Results - as soon as results are final. Sometimes you may have access to the results BEFORE your provider has seen them, but if there is a critical result, you will be contacted immediately. Sometimes test results may show outside of the normal range. Often, these out-of-range results do not affect your child's health.
- You can choose when you want to review results:
- Prefer to see results right away?
- Friends/Family may often help explain results, while others choose to search on the Internet for more information.
- Common medical terms can be found under Resources Menu.
- Your medical care team will be the best resource to explain any results as each patient is unique.

- Review after you hear from us. We will contact you within a few days after we have reviewed the results.

Why can't I see all my medical records in mySRH?
- Clinical Notes availability became active on November 30, 2020. Clinical Notes completed before this date can be requested using the Medical Record Request form under Health Menu.
- Other Clinical Notes may not automatically be available due to the sensitivity of the information or due to state or federal laws that prohibit auto-releasing (e.g., Research, Behavioral Notes, etc.).
- Results availability became active on October 1, 2016. Results finalized prior to this date can be requested using by Medical Record Request form under Health Menu.
- Other results may not automatically be available due to the sensitivity of the test (e.g., some genetic tests, etc.).
Why are certain test results not shared electronically via mySRH?
Your provider is able to determine which types of test results are able to be accessed through mySRH. Further, tests of a very sensitive nature are not released to mySRH.

Your mySRH information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

You will generally receive an answer within 1-3 business days. Please note that mySRH should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.
Return to TopCan I view a family member's health record in mySRH?
Yes you can. This is called Proxy access and allows a parent (or guardian) to log into their personal mySRH account, and then connect to information regarding their family member. Complete a Proxy Consent Form and return it to one of our medical facilities to request access to this convenient service.

Can I ask questions regarding a family member from my mySRH account?
MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

You may contact our mySRH Patient Support Line at 214-559-5020 to request a new, secure password. You may also click the "Forgot password" link on the sign-in page to reset your password online.
Return to TopCan you send me a new activation code as I have lost it, let it expire or did not receive it?
Contact us at 214-559-5020 and after we verify your information, a new code will be sent via U.S. Postal Mail. Privacy issues prevent us from e-mailing a new activation code to you.

Log into mySRH and from the left menu, go to the Preferences section and select the appropriate option.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all mySRH messaging is done while you are securely logged on to our website.

mySRH is owned and operated by TSRHC and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by mySRH.
Return to TopI was logged out of mySRH, what happened?
We aim to protect your privacy and security of your information. While logged into mySRH, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of mySRH. We recommend that you log out of your mySRH session if you need to leave your computer for even a short period of time.

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).
Return to TopMy activation code does not work, what should I do?
For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. If you still have problems, call our mySRH Patient Support Line at 214-559-5020.
